Medical Consent

I, as the parent or guardian of (player’s name), do hereby give my approval for their participation in any and all Upshur County Youth BASEBALL or SOFTBALL organization league activities. I hereby grant my permission to managing personnel or other organization league representatives to authorize and obtain medical care, at my expense, from any licensed physician, hospital, or medical clinic should the player become ill or injured while participating in organized league activities away from home, or where neither parent nor legal guardian is available to grant authorization for emergency treatment. I assume all risks, hazards incidental to my child’s participation, including illness associated with COVID-19 and transportation to and from the activities; and do hereby waive, release, absolve, indemnify and agree to hold harmless the local Upshur County Youth Baseball and Softball organization, the organizers, sponsors, supervisors, participants, and persons transporting the player to and from the activities, for any and all claims arising out of an injury, illness or death of/ to the player. I further agree to furnish certified birth documentation for the player, upon request by organization league officials, and to return upon request the equipment issued to the player in as good a condition as when received, except for normal wear and tear in organization league activities. I agree to follow ALL Health Guidelines for the return to play protocols. Including assisting coaches, administration, and the league with mentoring/guiding your child/children through these Health Guidelines. By Initialing below, that is my signature.

Code of Conduct

Upshur County Youth League Code of Conduct1. I / We hereby pledge to provide positive support, care, and encouragement for my child participating in the Upshur County Youth Baseball / Softball Leagues, by following the Code of Conduct.2. l/We will encourage good sportsmanship by demonstrating positive support for all players, coaches, umpires, and any league officials at every game, practice, or associated event.3. l/We will place the emotional and physical well-being of my child ahead of my personal desire to win.4. l/We will insist that my child plays in a safe and healthy environment.5. l/We will support coaches and officials working with my child, in order to encourage a positive and enjoyable experience for all.6. l/We will demand a sports environment for my child that is free from drugs, and alcohol and will refrain from their use at all youth sports events.7. l/We will remember that the game is for youth — not adults.8. l/We will do my very best to make youth sports fun for my child.9. l/We will ask my child to treat other players, coaches, fans, and officials with respect regardless of race, sex, creed, or ability.10. l/We will help my child enjoy the experience by doing whatever I can, such as being a respectful fan assisting with coaching or providing transportation, or assisting Upshur County Youth Baseball / Softball program as a volunteer.11. l/We will abide by all the rules and regulations of Upshur County Youth Baseball / Softball and know that I am responsible to read and know them.12. l/We agree to accept all decisions of the game officials, umpires, coaches, Board of Directors as being fair and called to the best of their ability.13. I/We will not engage in any kind of unsportsmanlike conduct with any official, coach, player, or parents such as booing and taunting, refusing to shake hands, or using profane languages or gestures.14. l/We agree to furnish the proper documents required by Upshur County Youth Baseball /Softball for my child to play (Failure to do so may result in the release of a player)15. Any disrespect from a parent/fan directed towards game officials, board officials, or any person volunteering and working the game, creating disturbance either in the stands or on the playing field, or has been ejected from the game will be referred by the Board of Directors for disciplinary action, and can result in game forfeiture.16. l/We agree to take responsibility for any actions that violate this Code of Conduct by a guest or relative. attending parent, or guardian.
